Likeness and voice release

Talent release form for your music video, photos and content

For everyone who appears in your music video, photo shoot or content: dancers, actors, extras, the crowd at a show you film up close. They let you use their image, name and voice for the project and its promotion, with clear limits. In English or Spanish, signed online without an account. Starting from a template uses no credits.

What the template includes

The project and the shoot date

What is being filmed or photographed and when, for example “the music video for the song ‘Fire’ by Luna”.

What it covers

The person’s image, physical likeness, name, professional name, voice, signature and performance, in the photos, footage and recordings of the project, raw or edited.

Permitted uses

The project itself on platforms, TV, cinemas and festivals; its promotion on social media, websites, press and release assets (trailers, stills, behind the scenes); the credits; and portfolio use. Anything else only if you write it in.

Excluded uses

Unless they agree in writing: advertising for brands, political or religious use, anything that harms their reputation, altering their image or voice with AI or using the footage to train AI, and selling the material as stock.

Territory and term

The world and five years by default, both editable. After the term no new uses, but the video already released can stay online.

Payment

Free of charge by default, or a fee you set, for example “USD 200 within ten days of signing”.

Approval of the edit

They see the edit where they can be identified before it is published and can object within 5 business days, only for harm to their reputation or privacy, not for artistic taste.

Credit, revocation and data

How they are credited (or not, if they ask), revocation only going forward, their personal data used only for the project.

Minors

Signed by their parent or guardian, with an adult present at the shoot and conditions suited to their age.

When to use it

  • Dancers, actors or extras appear in your music video
  • You are shooting photos with a model for the cover or the press kit
  • You film fans or friends up close at a show and want to use the footage in a clip
  • A minor appears in the video and you need their guardian’s signature

Before you sign: what to check

  • That the project is described precisely: which song, which video or which shoot
  • Whether you need uses beyond the project and its promotion, and that they are written in
  • The term and the territory, and what happens to the video once the term ends
  • Whether the person is paid, how much and when
  • How the person is credited, or that they asked not to be
  • If they are a minor, that their guardian signs and is present at the shoot

Hiring the director or photographer as well: music video contract template. When a guest artist sings on the song, their consent goes in the featured artist agreement, not here.

Frequently asked questions

Is the talent release form free?

Yes. On the Free plan you can use every template as often as you want; starting from one uses no credits. The Free plan includes 2 documents to sign online each month, and Pro has unlimited signatures. See the plans.

Who needs to sign a release for a music video?

Everyone who can be identified on camera other than the artist: dancers, actors, extras and people filmed up close. For minors, their parent or guardian signs. A crowd shot where nobody stands out is usually treated differently, but the rules depend on the country.

Does a release let me use their image in an ad?

Not with this template. Advertising for brands, sponsorships and anything suggesting that the person endorses a product are excluded unless they agree to it specifically in writing.

Can I use the footage to make an AI version of their face or voice?

No. The template excludes altering their image or voice with AI to create images, voices or performances they didn’t give, and using the footage to train AI, unless they agree in writing.

Does a release cover their music too?

No. It covers only the person’s image, name and voice. If they are a musician and you use their songs or recordings, that needs a separate agreement.
